Original Polish medical insurance booklet (Legitymacja Ubezpieczeniowa), dating from the 1943–1959 period.
This cloth-bound booklet retains its original worn canvas cover with faint printed lettering. The interior contains printed medical entry pages in Polish, including sections for physician notes, official stamps, and treatment records.
Visible entries include:
Medical consultation notes dated between 1943 and 1959
Physician stamps and handwritten signatures
Ophthalmology notation (“Okulistyka”)
Official ink stamps and administrative markings
The booklet retains its original affixed identification photographs with period cancellation stamps, adding strong personal and historical character to the piece.
A tangible example of WWII and post-war Polish social and medical documentation, suitable for collectors of Eastern European historical ephemera or transitional WWII/post-war material.
Condition:
Heavy age wear to the canvas cover with fraying along the edges and spine. The cover is structurally weakened at the spine and partially separated; when opened, it does not hold firmly and a visible gap appears between the cover sections, although it remains partially attached. Interior pages remain intact with staining, edge wear, and honest period handling marks. Photographs remain affixed.
